Address Change Notification

Please use the form below to submit any changes in your U.S. residential or mailing addresses or permanent foreign address to the ISO. The ISO will then notify SEVIS of the changes.

*Please note that this form will not update your address in HRMS or in GLACIER. You will still need to access those websites and make your address changes there as well.*

Students who are subject to Special Registration must also complete the AR-11SR Form and submit it directly to immigration at the following link. http://www.ice.gov/pi/specialregistration/index.htm

Note: If you have graduated and are currently on Optional Practical Training, please select "Post-Completion OPT" as your division instead of your previous UR division.
