F-1 OPT Reporting Form for Eastman Students

This form is intended for students or graduates of UR's Eastman School of Music only!

If you attended a different school or academic program at the University of Rochester, please use the F-1 OPT Reporting Form instead.

Employment information must be reported in SEVIS for all F-1 students who are authorized for Optional Practical Training (OPT). Students are required to submit details of their current employment activities and primary work location, as well as a valid email, phone number and residential address. The form below will allow you to report required information easily and quickly. Upon receiving the form, ISO will update your SEVIS record accordingly. In order to prevent reporting delays to SEVIS, please complete the form thoroughly.

Failure to report employment information may result in the termination of your SEVIS record, based on OPT Unemployment Limits.  Updates must be submitted within 10 days of any change. Please use the Eastman OPT Reporting Form as often as necessary to ensure that ISO has your most current information. If your OPT will expire or you will no longer maintain your F-1 status through University of Rochester, please use the Student Departure Form to notify us of your plans. If you have questions about this form, please send a message to ISO's OPT Mailbox.

Instructions

This form can be used to report employment that is either beginning or ending. To add or edit your employment activities, please distinguish between on-going employment or volunteering versus self-employment for less formal or short-term activities. In the Report Employment section, you can identify either paid employment or unpaid volunteer work and list the employer name, address, start date, and time status (part-time vs. full-time). You also need to include a detailed description of how this particular job or activity is related to the field of study that is listed on your I-20, which is a requirement of all F-1 OPT employment. In the Self-Employment in the Performing Arts section, you must acknowledge that the description provided is appropriate to what you are doing and give an estimate of the number of hours per week you are engaged in related activities. Again, you must include a detailed description of how this relates to the field of study listed on your I-20. You should keep a detailed record of your weekly activities, but do not need to report this information to ISO or to SEVIS. If you are ending or leaving a position that you previously reported to ISO, please use the Report End of Employment section. When appropriate, you can submit this form with multiple sections completed at once with all the relevant information.
