F-1 OPT Reporting Form for Eastman Students

This form is for F-1 students or graduates of the University of Rochester’s Eastman School of Music only! If you attended a different school or academic program at the University, use the standard F-1 OPT Reporting Form instead.

Employment information must be reported in SEVIS for all F-1 students who are authorized for Optional Practical Training (OPT). You are required to submit details of your current employment activities and primary work location, as well as a valid email, phone number, and residential address. The form below will allow you to report required information easily and quickly.

Failure to report employment information may result in the termination of your SEVIS record, based on OPT unemployment limits. Updates must be submitted within 10 days of any change.

The Eastman OPT Reporting Form below allows you to report required address and employment information. Upon receiving the form, the International Services Office (ISO) will update your SEVIS record accordingly. However, if the information provided is incomplete or inaccurate, this will create delays for your required SEVIS reporting. Please complete the form thoroughly!

Use the Eastman OPT Reporting Form or the SEVP Portal as often as necessary to ensure that ISO has your most current information.

If your OPT will expire or you will no longer maintain your F-1 status through University of Rochester, please use the Student Departure Form to notify us of your plans.

Instructions

Use this form to report new or updated address and employment details.

If you are only reporting residential address information, please type N/A in all fields related to employment.

Due to the unique nature of performing arts, Eastman students on OPT may be participating in regular, ongoing employment; a combination of gigs and other short-term activities adding up to 20 hours/week; or both.

To report any regular, ongoing employment, use the Report Employment section of the form below. You can identify either paid employment or unpaid volunteer work and list the employer name, address, start date, and time status (part-time or full-time). You also need to include a detailed description of how this particular job or activity is related to the field of study that is listed on your I-20 (Major 1 and/or Major 2), which is a requirement of all F-1 OPT employment.

To report a combination of gigs and other short-term activities, use the Self-Employment in the Performing Arts section of the form below. You must acknowledge that the description provided is appropriate to what you are doing and give an estimate of the number of hours per week you are engaged in related activities. Again, you must include a detailed description of how this relates to the field of study listed on your I-20. You should keep a detailed record of your weekly activities, but do not need to report this information to ISO or to SEVIS.

If you are ending or leaving a position that you previously reported to ISO, please use the Report End of Employment section.

When appropriate, you can submit this form with multiple sections completed at once with all the relevant information.
