F-1 STEM Extension OPT Reporting Form

Students pursuing a 24-month STEM Extension must adhere to rules and requirements during the STEM period, including significant reporting obligations to ISO. At all times, our office must have accurate information regarding your activities and reliable contact information. Updates must be submitted within 10 days of any change.

Upon receiving the form by email, ISO will update your SEVIS record accordingly. However, if the information provided is incomplete or inaccurate, this will create delays for your required SEVIS reporting.

Failure to maintain qualified employment could result in the termination of your SEVIS record, based on STEM Unemployment Limits. If your STEM Extension OPT will expire or you will no longer maintain your F-1 status through University of Rochester, please use the Student Departure Form to notify us of your plans.

Alternative STEM Reporting Shortcuts:

Student Departure Form: To report a change in immigration status or plans to depart the U.S. (either during or following completion of STEM authorization period).
Form I-983 Training Plan: To submit annual evaluation details OR report a change in STEM employment details.
