Prearrival Forms & Instructions

Whether you are a newly-admitted student coming to Rochester for the first time, or a former student returning to UR after an absence, the ISO needs some basic information in order to issue you a Certificate of Eligibility (I-20 for F-1 students, DS-2019 for J-1 students). You will need to complete the New Student Request Form linked below, and then return the form to your department representative.

Please make sure that you fill in the request form completely and accurately. Please attach photocopies of any existing immigration documents you may have (passport, current visa, I-94 card, I-20 or DS-2019). If financial support will be provided from a source other than UR, please attach original documentation for proof of funding.

New Student Request Form (pdf) — Used to request an initial Certificate of Eligibility for all international students. This form is a fillable form, so please enter your information before printing if possible.

Once you receive your I-20 or DS-2019 from the University, please read the ISO's Prearrival Information. It contains invaluable information regarding the SEVIS fee required for new students and the Form I-901, applying for a visa and preparing for your departure and arrival in Rochester.
